Service is provided at the medical/surgery office or patients home for physicians or surgeons who do not have a Registered Nurse on staff for hydration services. *The Surgeon/Physician is required to sign "Independent Contractor Agreement" to contract Mobile Nurse Crystal to provide services, complete "Client Health History" of your clients full medical/surgery history and nursing orders, and client sign "Promotional Consent Form". All forms and consents will be emailed to you after booking. All Client Information is HIPPA protected.* -Travel and Parking Fee $50 Required: Medical Doctors must have their own medications & supplies to use for ALL clients.
Cancellation Policy
Non-refundable Fee for ALL Services once Booked. If need to cancel, the services will be credited to your account for future services.
